Housebreaking a dog....?

I've been crate training her since I got her but the crate doesn't seem to stop her from urinating/defecating on her bed. It's not an overly large crate but I don't know what else to do. I've tried cutting off her water at 6 (her bedtime is at 9). Didn't make any difference. Any additional ideas are most welcome.
